Fill out the form below to start the membership renewal process. To complete the form you will need the following information:

  • A spreadsheet with your list of programs and schools you visited in the past year. Please note that for membership renewals, the membership year is considered to be July 1 - June 30. Please use this period of time when answering the questions below related to Site Visits and Collaboration Calls, BOTB Calls, Check-In Calls, and Committee Participation Calls.

    • Each of the visits should be unique, with no repeat visits during the membership year (July 1, 2024 - June 30, 2025). Up to 20% can be virtual visits. At least 50% must be therapeutic schools or programs. This can include local day schools or non-residential options in your own geographic area.

    • Professional Members must list 25 visits and Associate Members must list 30 visits.

  • Any new relevant degrees or certifications

  • Any updates to your general information (email, website, etc.), business, and focus of practice
